Incoming Senior Students

Students need to get a second meningococcal (ACWY) shot before they start their senior year of high school. If your student has already received this shot or has documentation, you can send or fax the records to the health office. The shot can be given anytime after your student turns 16. If you're not sure whether your student has had the shot or turned in the papers, please call the health office at 736-7040.

Upcoming Immunization Clinic

Clay County Health Department offers a fall and spring immunization clinic at LHS. For more information please contact the health office.

Health Record Update 

Each year, every returning LPS student must complete a Health Record Update online. This form will give consent for the nurse to administer Tylenol.

If your child will be taking medication other than Tylenol at school, please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy, please complete an Action Plan, Medication Permission Form, and Self Administration Form.
